The following application is for MDA local components to submit a project for consideration to receive a $5,000 PR Grant to use for promoting oral health in their communities. Please complete all fields and click the submit button when finished in order to be eligible for entry. Components must submit one application per project. Applying for the MDA PR grant program does not guarantee component will receive a grant.
As of Nov. 4, logging into the MDA site has changed with the launch of the new MDA database. Your previous password will no longer work for logging into the new MDA database. Follow these steps to reset your password and get logged in:
• Enter the email you previously used for logging in and follow the prompts.
• You’ll be asked to set your new password.
• Once you’re logged in, you can click My Profile in the top right corner to update your profile, renew membership, and more.
